Portal Forms is an online form creation, response collection, reporting, and file upload service. Users may create forms, publish links, collect responses, view summaries, and manage uploaded files based on their account plan.

2. Account Responsibility

• You are responsible for all activity under your account or workspace.
• You must keep login credentials secure and not share access improperly.
• You must ensure that your forms comply with applicable laws and institutional rules.

3. Form Owner Responsibility

The form owner is responsible for the content of each form, the purpose of data collection, the questions asked, and how collected responses are used.

4. Platform Rights

Portal Forms may review, restrict, suspend, or remove forms that violate these policies, are reported for abuse, or create security or legal risk.
